We are looking for an experienced, compassionate and inspiring leader to join our Clinical Services leadership team as our Family Support and Transition Lead.

This is an exciting opportunity to shape and develop family support, safeguarding and transition services, ensuring children, young people and their families receive coordinated, person-centred support throughout their journey with Haven House.

Hours: 35 hours per week

Salary: £42,025–£49,199 per annum, dependent upon experience.

About the role

This is a varied and rewarding leadership role bringing together family support, safeguarding and transition into adult services.

Working closely with the Therapies & Wellbeing Lead and Director of Clinical Services, you will:

  • Lead and support our Family Support Team.
  • Act as Deputy Safeguarding Lead, helping to embed a strong safeguarding culture across the hospice.
  • Lead the development of transition pathways for young people moving into adult services.
  • Work closely with families, clinical colleagues and external partners to coordinate complex support.
  • Champion family participation and co-production, ensuring the voices of children, young people and families influence service development.
  • Contribute to quality improvement, organisational learning and the continued development of our services.

About you

We are looking for someone who is:

  • A registered health or social care professional (Social Work England, NMC, HCPC or equivalent).
  • Experienced in safeguarding, family support and multi-agency working.
  • Passionate about improving outcomes for children, young people and families.
  • An experienced and supportive leader who enjoys developing others.
  • Confident building strong partnerships across health, social care, education and the voluntary sector.
  • Committed to compassionate, high-quality, family-centred care.
